Position Summary
The Lead Care Manager delivers compassionate, whole-person care through outreach, education, comprehensive care coordination, and support services under the CalAIM Enhanced Care Management (ECM) program. Working directly with enrolled members primarily in their homes, this position addresses social determinants of health and empowers vulnerable populations to navigate complex, fragmented healthcare systems safely.
Core Responsibilities
- Assessment & Care Planning: Interview potential program members and review clinical and eligibility data for enrollment into ECM. Complete comprehensive member assessments and formulate, evaluate, and update individualized, goal-oriented Care Plans while providing continuous client education.
- Member Care & Coordination: Routinely check in with assigned members via face-to-face home visits, phone interactions, or telehealth modalities. Accompany members to critical clinical appointments as needed to support adherence. Coordinate with healthcare providers, community-based services, and interdisciplinary partners to establish seamless care and avoid duplication.
- Safety & Crisis Management: Actively identify and respond to critical concerns involving member safety, physical pain, or mental health stability; formulate individualized crisis safety plans as required. Strict adherence and compliance with mandated reporting laws regarding child, adult, and elder abuse.
- Resource Linkage & Referral Tracking: Maintain up-to-date knowledge of local community resources (including housing, nutritional assistance, transportation, and financial support) to link members to appropriate services. Monitor and perform timely follow-up on external referrals to ensure continuity of care and that client needs are fully met.
- Documentation: Record all member encounters, care coordination efforts, and clinical progress notes directly into the Electronic Health Record (EHR) system in an accurate, compliant, and timely manner.
Qualifications & Requirements
- Education & Certification: High School Diploma or equivalent required. A Medical Assistant (MA) or Certified Nursing Assistant (CNA) diploma is strictly required for compliance. Bachelor’s Degree in Social Work, Psychology, or Sociology preferred.
- Experience: Minimum of 2 years of professional experience in a healthcare-related role. Experience in care coordination, social work, or community health programs is strongly preferred.
- Travel & Licensing: Must possess a current, valid Driver’s License, a clean driving record, and proof of active auto insurance. Absolute willingness and ability to travel extensively throughout the designated county to perform home visits (90% of work is field-based).
- Skills: Strong computer proficiency including Microsoft Office suite, email systems, and virtual communication platforms. Excellent interpersonal and communication skills. Proven ability to work independently, prioritize heavy field tasks, and follow through on deliverables. Bilingual fluency in Spanish and English is a strong plus
Work Schedule & Location
- Location: Field-based (90% field travel throughout the local county) /Remote-Hybrid
- Schedule: Monday-Friday 8am-5pm
- State Constraint: Because St. Vincent Preventative Family Care is a California-based employer, all customary work must be performed within the state of California.
Compensation & Benefits
- Hourly Range: $25.00 – $30.00 per hour (Dependent on Experience) ● Classification: Full-Time, Non-Exempt
