Position Summary

Community Health Worker and become a vital link between healthcare providers and the communities we serve. In this dynamic role, you will empower individuals to achieve better health outcomes through personalized support, education, and resource navigation. Your enthusiasm and compassion will help bridge gaps in healthcare access, promote wellness, and foster healthier communities.

Core Responsibilities

● Community Education & Outreach: Educate diverse individuals and local communities on critical health topics, programs, and available care resources. Conduct strategic outreach activities to actively promote community health programs and enrollment services.

● Navigation & Case Management: Assist individuals in navigating complex healthcare systems and gaining direct access to appropriate medical and social resources. Connect individuals with multi-tiered resources including healthcare, housing, and social services, providing structured case management support. Manage a defined caseload and coordinate incoming client referrals directly from health plans.

● Direct Client Advocacy: Provide essential social support and dedicated advocacy for individuals facing severe physical, behavioral, or social health challenges. Conduct structured home visits to evaluate client needs, assess environments, and deliver targeted health education. Utilize motivational interviewing techniques to encourage positive behavior change and self-advocacy.

● Care Collaboration & Records Maintenance: Collaborate closely with healthcare professionals and multidisciplinary teams to coordinate comprehensive care for clients. Collect operational data and maintain precise, accurate, and completely confidential records of all client interactions and documentation.

Qualifications & Requirements

● Education: Bachelor’s Degree in Social Work, Psychology, or Sociology preferred.

● Certification: Community Health Worker (CHW) Certification is highly preferred but not required.

● Experience: Minimum of 2 years of professional experience within the Community Health Space required. Proven experience conducting outreach and successfully enrolling patients into formal managed health plan programs is required.

● Skills & Knowledge: Solid knowledge of medical terminology and general healthcare systems. Demonstrated ability to communicate effectively and with deep cultural sensitivity with individuals from highly diverse socioeconomic backgrounds. Proficient in conducting public speaking engagements and facilitating group discussions. Familiarity with addiction counseling practices and substance use disorder resources. General understanding of Medicare guidelines and eligibility requirements. Strong capability in data collection and strict documentation maintenance.

Work Schedule & Location

● Location: On-site/Hybrid

● Schedule: Monday-Friday 8am-5pm

● State Constraint: Because St. Vincent Preventative Family Care is a California-based employer, all customary work must be performed within the state of California.

Compensation & Benefits

● Hourly Range: $25.00 – $30.00 per hour (Dependent on Experience) ● Classification: Full-Time, Non-Exempt

● Sector-specific familiarity (experience within healthcare, social services, or CalAIM frameworks)
