CASE AIDE
Position Overview: We seek many dedicated and empathetic Case Aides with livedexperience
with navigating social services and medical coordination. Familiarity with the challenges of
working in a residential/motel setting. The ideal candidate will possess Certified Nursing
Assistant (CNA) qualifications but will not be required to perform tasks such as changing. A
license is not required. The Case Aide will support clients who are largely independent and
require assistance with daily activities, health monitoring, and motivational interviewing.
Key Responsibilities:
Daily Client Interaction:
• Conduct daily temperature checks and health assessments.
• Perform first aid as needed.
• Engage clients in motivational interviewing to support their recovery goals.
• Assist clients with the process of moving into permanent housing.
• Provide emotional support and encourage client self-sufficiency.
Record Keeping:
• Maintain accurate and detailed notes and logs for each client daily.
• Ensure all documentation is completed in a timely and professional manner.
• Adhere to HIPAA regulations to ensure client confidentiality and privacy.
Health and Safety:
• Monitor clients for potential exposure to illnesses and take appropriate precautions.
• Ensure the cleanliness and safety of the motel setting.
Client and Visitor Support:
• Greet and assist clients visiting the office who are not currently in the program.
• Provide information and support to walk-in clients seeking assistance.
• Assist clients with navigating available resources and services.
Clerical Tasks:
• Perform clerical tasks such as filing, data entry, and managing client records.
• Assist with scheduling appointments and coordinating services for clients.
Facility Maintenance:
• Ensure the facility is kept clean, safe, and organized.
• Report any maintenance issues or safety hazards to the appropriate personnel.
Communication and Reporting:
• Report any concerns or significant changes in client health or behavior to the Lead Case Aide.
• Participate in team meetings and contribute to care planning.
