Harmony At Home is looking for a Family First Lead Care Advocate to join our team!

The Family First Lead Care Advocate plays a key role in providing comprehensive, person-centered Enhanced Care Management (ECM) services to high-risk children and families enrolled in the Harmony At Home Family First program who are Medi-cal eligible. This position is responsible for managing a caseload of 40-50 ECM members, developing individualized care plans, and coordinating services that address both medical and social determinants of health. The Lead Care Manager acts as the primary point of contact for their assigned members and works collaboratively with internal team members, community-based providers, and healthcare professionals to ensure whole-person care based on MCP requirements Additional tasks dependent on duties and CCAH requirements.

 

ESSENTIAL JOB FUNCTIONS, DUTIES AND RESPONSIBILITIES

CASE MANAGEMENT:

  • Manage a panel of 40-50 ECM members, including children and families with complex needs.
  • Meet with each member at least once per month in person or virtually to build rapport, assess needs, and review/update care plans.

CARE PLANNING & COORDINATION:

  • Conduct comprehensive assessments to identify medical, behavioral, social, and environmental needs.
  • Collaboratively develop, implement, and monitor individualized care plans based on member goals and priorities.
  • Navigate health insurance benefits and coordinate services across physical health, behavioral health, and social services systems.

DOCUMENTATION & COMPLIANCE:

  • Accurately document all contacts, care coordination activities, and outcomes in the CCAH and case management platforms.
  • Ensure timely submission of notes and billing entries in alignment with program requirements and HIPAA regulations.
  • Maintain up-to-date and complete records that meet all audit and compliance standards.

COLLABORATION & COMMUNICATION:

  • Serve as the lead care manager for assigned members, coordinating with Family First Case Advocates to support dual-enrolled members.
  • Facilitate case conferences with care teams, which may include family members, behavioral health providers, educators, and healthcare professionals.
  • Attend team meetings, training, and supervision sessions as required.

OUTREACH & ADVOCACY:

  • Engage families and youth in culturally competent and trauma-informed ways.
  • Advocate for services and resources on behalf of members to address housing, food, transportation, education, and employment needs.
  • Assist members in overcoming barriers to care and navigating complex systems.

COLLABORATION AND COMMUNICATION:

  • Attend mandatory training and meetings for program implementation and improvement.
  • Maintain clean and well-stocked office space, adhering to office procedures and expectations.
  • Represent Family First and Harmony At Home at community events or meetings to promote the program’s mission and services.
  • Conduct outreach presentations to promote the Family First program mission and services.
  • Collaborate with the Family First team, funding partners, and referral sources.

GOOGLE CALENDAR MANAGEMENT:

  • Maintain an updated Google calendar with all scheduled program-related activities, meetings, and events.
  • Ensure timely communication and coordination of schedules with program staff, participants, and external partners.

PROGRAM DEVELOPMENT:

  • Assist in the development and implementation of programs and activities that promote family well-being and child development.
  • Collaborate with the Program Manager to plan and execute program initiatives.
  • Conduct outreach and recruitment efforts to engage families in the program.
  • Assess and communicate program needs with the Program Manager.
  • Perform additional duties as required to support program improvement and implementation.

 

REQUIRED SKILLS:

  • Passionate about working directly with youth and at-risk populations
    Strong public speaking and group facilitation skills
  • Ability to manage multiple priorities within a constant changing environment

COMPUTER/SOFTWARE/TECHNICAL SKILLS REQUIREMENT:

  • Extensive experience of computer applications, including: word processing, relational databases, spreadsheets, and extensive knowledge of the Internet.
  • Experience in the following computer programs preferred: Google Docs, Google Sheets, Google Slides, Google Drive, Excel, PowerPoint, Word, Canva, and Outlook.
  • Ability to apply basic mathematical, reasoning, and problem solving concepts.

LANGUAGE SKILLS REQUIREMENT:

  • Ability to read and write in the English language proficiently.
  • Ability to read and speak Spanish preferred.
  • Strong verbal, written skills and excellent communication skills.
  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or government regulations.
  • Ability to effectively present information and respond to questions from supervisors, co-workers, volunteers, members, donors and the general public.
  • Strong public speaking and group facilitation skills
  • Ability to manage multiple priorities within a constant changing environment
  • Knowledge and comfort discussing sexuality and promoting family planning
  • Excellent decision making and problem solving skills must be organized, detail-oriented, energetic, and flexible.
  • Ability to be empathetic while maintaining professional boundaries.
  • Knowledge of positive youth development concepts, approaches, and research.

EDUCATIONAL / CERTIFICATION / EXPERIENCE REQUIREMENTS:

  • Bachelor’s or master’s degree Preferred in a relevant field (counseling, social work, public health, psychology, health education) or related work experience.
  • Work experience will be accepted in lieu of Bachelors Degree
  • One to two years related work experience.
  • Experience in positive youth development, reproductive health, at-risk youth
  • This position requires some travel.
  • Experience with infants and small children.

 

ADDITIONAL ESSENTIAL FUNCTIONS, REQUIREMENTS, COMMENTS:

  • Must be available to work daytime, evenings and weekends as necessary to completely perform position requirements.
  • Must maintain an excellent attendance record.

HOURS

  • Up to 40 hours per week. Some evenings and weekends as needed.

*Hours are subject to change to meet the needs of Harmony At Home and Family First Program and CCAH requirements.

 

SALARY

  • Starting salary is $28.00 per hour
