Job Summary:
The Program Manager at Mysti’s Adult and Family Services is responsible for the development,
implementation, and oversight of comprehensive care management programs, incorporating
trauma-informed care and housing-first principles. The manager will ensure the integration of
service delivery across internal and external programs, including recuperative care, caregiving
services, and housing solutions, all while maintaining client-centered and culturally sensitive
practices.
Key Responsibilities:
1. Program Development & Management:
o Develop and implement programs aligned with the organization’s mission, with a
focus on housing-first approaches and trauma-informed care models.
o Ensure integration with external and statewide resources, including Enhanced
Care Management (ECM) and Community Supports services.
o Oversee the referral process, case management, and coordination with various
health and social service providers.
2. Service Delivery & Coordination:
o Coordinate across programs, including housing services, caregiving, recuperative
care, and community supports, to create seamless client transitions.
o Collaborate with local and state agencies to ensure that clients receive
comprehensive and integrated care.
o Monitor case management processes, ensuring that service delivery meets both
trauma-informed and housing-first care standards.
3. Staff Management & Training:
o Supervise and mentor program staff, including case managers, social workers, and
administrative support.
o Provide ongoing training on trauma-informed care, care coordination, and
housing-first principles to ensure staff development.
o Implement continuous improvement initiatives based on staff and client feedback.
4. Client Care Planning:
o Oversee the creation and execution of client-centered care plans, ensuring they are
culturally and linguistically appropriate.
o Utilize data and evidence-based practices to monitor client outcomes and adjust
care plans as necessary.
o Foster relationships with internal teams and external providers to ensure the
delivery of care aligns with client goals.
5. Compliance & Reporting:
o Ensure all programs are compliant with federal, state, and local regulations,
including Medicaid/Medi-Cal guidelines.
o Prepare regular reports on program performance, outcomes, and financial
management.
o Ensure accurate documentation of client care plans, assessments, and services
rendered.
6. Community & Stakeholder Engagement:
o Engage with community partners, including Continuum of Care networks and
local service providers, to expand resources for clients.
o Represent Mysti’s Adult and Family Services in external meetings, ensuring
alignment with community-wide efforts to address homelessness and other social
issues.
7. Data Management & Analysis:
o Utilize data systems for tracking program performance, client outcomes, and
service utilization.
o Report outcomes to funders, stakeholders, and executive leadership,
recommending program adjustments as needed based on findings.
Qualifications:
• Education: Bachelor’s or Master’s degree in Social Work, Public Health, Human
Services, or a related field.
• Experience: Minimum of 5 years of experience in program management, particularly in
housing services, trauma-informed care, or healthcare services.
• Skills: Strong leadership, communication, and organizational skills; experience with care
management platforms and data tracking.
• Knowledge: Familiarity with Cal AIM, Enhanced Care Management, Medi-Cal
programs, and community support systems
